Life Insurance Information

The life insurance program provides both life insurance coverage in the case of death and Accidental Death and Dismemberment coverage in the case of certain dismembering injuries or accidental death. Please click on the below link for complete information on this benefit and your options for additional coverage.

How to View Your Current Coverage and Apply or Drop  Supplemental Coverage

If you would like to increase or decrease your Life Insurance coverage, please log into your PeopleSoft HR and to the following links:

• Employee Self-Service>Benefits>Benefits Home>Life Insurance
• Click on the "Edit Your Coverage Amount" button
• The correct current insurance coverage amount, premium information, and appropriate options (to add or drop your supplemental coverage) will be available to you.

Any increase in coverage requires Evidence of Insurability (EOI). If an EOI is required, once you have submitted the request on Employee Self Service, you receive an email within 7 business days from The Hartford with instructions to their website to complete the questionnaire.  Once completed, the The Hartford will notify you and the Benefits Office by mail as to their decision.

If you have questions or would like to follow up on your Life Insurance Application status, please contact The Hartford directly at 1-877-663-4278. 

Designating Your Beneficiary For Life Insurance Coverage

You may change your beneficiary for life insurance at any time. View and make changes to your beneficiary designations for life insurance online by logging into your PeopleSoft HR and click on the following:

• Employee Self Service >Benefits Home>Life Insurance
• Select "Edit Your Beneficiary"

You may also print the form by clicking on the link below return it the Benefits Office, Bldg 400B.
