Instructions:
1.BUDGET
Use current B&R number
from http://www.cfo.doe.gov/EFASC/FRAD/BARC/index.htm
.
2.CONTRACTOR NUMBER
The Contractor Number (also known as the FWP Number) should
conform to the following format:

3.DATE PREPARED
The date will be provided on page 1 of the FWP. However, on the subsequent pages you need to
type the date prepared as yyyymmdd on all continuation pages of the
proposal. All signature dates should also be same as date prepared during
Budget Submission. For FY 2011, Budget Submission date prepared is March 1,
2009 (20090301).
4.TASK TERM
Enter the start and estimated completion date of the
proposed work if known. Use a two-digit format for month, day; for
example, 10/01/xxxx. Dates must be entered even if they are only estimates.
5.
Work Proposal Numbers are no longer used.
6.
Work Authorization Numbers should be entered as **CH13# .
(Where ** are first two letters of
the B&R code and the
last letter '#' is the last digit of the actual fiscal year the budget was
submitted. Example: For a KB project for the current fiscal year the Work
Authorization Number will be (i.e.: KBCH139 ), remember no
hyphens.
7.TITLE
Insert the descriptive title of the work being proposed in
this space. Please make sure that this title agrees exactly to the title on
your FWP Master List during Budget Submission (page 4 in above Related
Documents list).
8.PRINCIPAL INVESTIGATOR(S)

The Budget Office during budget submission prepares this
page (financial information). If required by DOE, any significant
milestones/product deliverables necessary for completion of the project should
be provided (use the "Milestone Attachment Form" from Page-2). This
should include the "Milestone Schedule" column and "Proposed
Schedule" column (month and year).
CONTINUATION PAGES:
Complete the following information on each continuation
page:
TITLE descriptive title of the proposal (same as #7).
PAGE NUMBER (starting with #3 for continuation page)
DATE PREPARED Same as Item 3 on Field Work Proposal
"Summary" Page.
B&R NUMBER
BOTTOM MARGINS In order to complete a paragraph, up
to two lines may be added beyond the one-inch margin on continuation page. The
following instructions are in the exact form to be followed. Performa
statements (highlighted) should be used where appropriate:
An FWP should be kept as brief as possible. Bullets
or phrases are preferred over sentences where possible. Contact
program offices directly for guidance on the amount of detail required.
Item No: 22. Detailed Attachments
a. Purpose. Supply a short (few line)
bulletized statement of goals for FY 2009-2011. In addition, the
following sentence must be added, to every FWP, either in this section or in
Item No. 18. "This work effort may support at a minimum level or
concurrently, as appropriate the Technology Transfer and Science Education
missions of the Department of Energy."
b. Approach. For new work, supply a short
statement of background/approach, and a list of key personnel where
known. For ongoing work supply only a short list of key personnel in
format: A. Jones (100%); B. Smith, Post doc (50%).
c. Technical progress. Supply a few lines highlighting
the previous calendar year. Include the number of publications
produced in prior years (a list should be provided to the program office upon
request, but separate from the field work proposal).

d. Future accomplishments. This is the heart of the technical proposal.
In a few paragraphs the author should give adequate information for the program
manager to judge the worth of this proposal. Include any special facility
requirements. Also include a justification of any significant changes to
the level of effort. Address any extraordinary mission critical needs for
capital equipment. Special facility requirements may be discussed in this
section. The format of this attachment for proposals to be peer reviewed should
be negotiated directly with the program office.
e. Relationship to other projects. Include only if collaborations or
coordinated efforts are important to accomplishing the proposed work.
f. Explanation of milestones. Should be linked to Item 26, and
should be appropriate for the type of work. Purpose is to assist program
manager to determine value of the proposal and to track progress of funded
work.
g. Capital Equipment Request
Summary. Include only if Capital Equipment
is included in the Field Work Proposal.
h. Other.
Do not fill in this section unless author believes it is essential to include
additional information.
GENERAL STYLE GUIDE INFORMATION:
Note: Most of the FWP form is preset for your convenience.
1.
Use single space.
2.
Font is Arial 10 in most
cases.
3.
Format is MS WORD
4.
Use impersonal, passive
grammatical construction throughout the proposal.
5.
Keep all parts of a name
together on one line.
6.
Keep FY 20xx together on one
line.
7.
Place periods and commas
inside closing quotation marks.
8.
Place colons and semicolons
outside closing quotation marks.
9.
When the element name is
spelled out, e.g., "carbon 14," a hyphen is not required.
However, if the element name is used as a modifier, a hyphen should be
used. If the chemical symbol is used and the isotope is to be
specified, the mass number should be superscript prefix.
10. Hyphenated words should not appear in
more than two consecutive lines.
11. Use $600,000 when referring to thousands
of dollars in text not $600K.
12. Underline Latin expressions, e.g., in
vivo, in situ, ab initio.
13. Some typical problem words to be written
as follows: beam line, sitewide, downtime, turnaround, FYs, infrared,
leadtime, FTEs, onsite, offsite.
14. Periods are not necessary after
INFORMATION CONCERNING SUBMISSION OF FWP TO BUDGET
OFFICE:
o The principal investigator is responsible
for the accuracy of the proposal. The proposal should
be proofread to ascertain that it is clearly written and grammatically correct.
Prepare a Proposal Information Questionnaire (PIQ) form for every proposal to
cover the disclosure statement regarding sensitive materials or work, as well
as the use of human or animal subject. If the answer to any part of No.5
on the PIQ form is "yes", complete a
o Submit a signed original of each proposal
along with a signed original PIQ to JoAnn M Durham (X8074), Bldg. 460 Budget Office. In addition, you must forward an electronic
version (in Word) via email to jdurham@bnl.gov as soon as possible (draft form
is acceptable), along with each Program Manager(s) name, email address(es) and
the FWP name/number of each proposal they should review. Please use the FWP number as the file name
and also in the Subject line in the email.
o In a folder, insert the original proposal
along with the original PIQ in the inside right pocket folder with a label
marked "ORIGINAL." No additional copies are needed.
o Prepare a label with the following
information and attach the label to the outside, upper, right hand corner
of the folder:
