Operational Readiness Review Checklist

  1. Obtain Building Access:
    • Building Safety Orientation is required for anyone that will be residing in 735 and/or doing equipment set up or spending significant time in the building.
    • Offered every Tuesday and Thursday @11:00 main lobby
    • JTA CFN 09 is required for any hands on work
    • JTA CFN 08 is required for administrative work
  2. Arrange for equipment delivery:
    • Keep operations staff informed of the expected arrival of equipment to building 735. Notify operations staff of any schedule changes or special equipment (i.e., rigging) needed for delivery/set up in laboratories.
    • Keep purchasing agent involved in any correspondence with vendor and significant schedule changes
  3. Assist in development of Work Permit for any vendors/installers:
    • Coordinate through Building Manager, preferably prior to arrival of vendors.
  4. Set-up and accept equipment to specifications:
    • Notify operations staff of any necessary facility changes (electric, air, water, cryogenic, gas etc) as early as possible. Failure to alert the operations staff early in the installation process can cause considerable delays.
    • Equipment acceptance documentation is required, contact Chris Porretto.
  5. Identify hazmat inventory:
    • Notify ESH Coordinator of any chemicals, gasses or projected wastes necessary for the set up period
    • Order materials or arrange transfer from existing labs
    • Complete necessary CMS transfers. Contact operations staff or CMS team for assistance if needed.
  6. Temporary Procedures:
    • Develop any required temporary procedures, i.e., cryo filling, equipment operation etc. These procedures should be attached to the work permit for installation.
  7. Signage:
    • Obtain and post any identified signage i.e, emergency placards (yellow background) for lab doors, satellite accumulation areas, sink discharge labels, cryo filling labels etc. Coordinate this with the operations staff members.
  8. Participate in any ESH high hazard reviews:
    • Cryogenic Systems
    • Flammable Gas Systems (CVD’s)
    • High Pressure/vacuum Systems
    • Lasers
    • Provide necessary documentation to appropriate committee and follow-up on corrective actions.
  9. Draft Experimental Safety Review:
    • Use electronic form on web
    • Prepare overview presentation for ESR Committee
    • Present ESR to Committee and walk down equipment
    • Make any necessary changes to ESR and equipment/facility
    • Complete any additional identified training (beyond JTA CFN 09)
  10. Complete Operational Procedures:
    • Equipment Operations
    • CFN Operations Safety Awareness form (checklist of equipment/facility hazards to orientate users).
  11. Establish any necessary Satellite Accumulation Areas:
    • Postings in area
    • Secondary Containment
    • Notify operations staff of satellite accumulation areas
  12. Operational Readiness Review (independent laboratory committee)
    • Participate in walk down (area scoped by ESR)
    • Provide any documentation, training records, procedures etc as requested
    • Make any changes to equipment/procedures required by committee
  13. Obtain Management Approval to operate
    • Update emergency postings on lab doors to “green”
