Back to Work Procedures, notes, etc.
In accordance with Federal Regulation IOCFR851, a Back to Work (BTW) slip
is required for the following circumstances:
- Illnesses or injuries causing absences from work for 5 consecutive
days or more or the equivalent for workers with alternate work schedules;
and/or
- Procedures or treatments that would negatively affect the employee's
ability to perform in a safe and reliable manner.
- For all occupational injuries or illnesses, BTW slips are
required for any period of lost time, including sick days, doctor visits,
physical therapy, compensation hearings, consultations or independent
medical examinations.
Personal notes from a personal physician should be presented at OMC where
the professional staff will review them and file in the medical record,
since these notes may contain personal medical information.
