Dr. Gary
Welch received his doctoral degree in physical therapy from Stony Brook
University.
He has additional certifications beyond being a licensed
Physical Therapist in NYS: Certified Functional Manual Therapist,
Certified Orthopedic Manual Therapist, and Certified Functional Capacity
Evaluation.
Hours, Location and Contact Information
- Location: Building 490 (Medical Research Building). Enter
through the doors to the Occupational Medicine Clinic to check in. Gary
Welch can be found in Room 5-305.
- Hours: Mon, Wed, Thurs, Fri 8:00-4:30 p.m.

Eligibility
- BNL employees who qualify for insurance benefits are eligible to receive
physical therapy.
- A doctor's prescription recommending physical therapy is required in
order to be seen for physical therapy at BNL.
Scheduling Appointments & First Visits
Call 631-344-3328 to schedule appointments. Save time at OMC download the Intake Form
(doc) for Physical Therapy & bring it with you to your first visit. You must
also bring a written prescription from your doctor who is ordering physical
therapy.
On the day of your first visit, come through the Occupational Medicine
Clinic in order to check-in. Please be prompt for all physical therapy
appointments.
