Daily Pre-entry Screening Process for Employees

As the Lab resumes operations, all employees scheduled to come to work on site must complete a daily pre-entry screening to determine if you may have been exposed to COVID-19. All staff and guests will be asked to check their temperature and review a set of health-related questions on a printed card before leaving home.

If you answer YES to any question

Contact the Occupational Medicine Clinic (OMC) at Ext. 3670 (631-344-3670) before coming to work. If it's before 8 a.m., leave a voice message and an OMC nurse will return your call. Also, call your doctor if you have fever/symptoms.

If you answer NO to all questions

Proceed to work and be sure to put the printed card face-up on your vehicle dashboard so an officer stationed at the Main Gate booth can see it.

If you have a printer at home, you can download (link above) and print a copy for your first day returning to work. You can then exchange that copy at the Main Gate for one printed on heavier card stock. If you are not able to print your own copy of the card, an officer stationed at the Main Gate booth will ask if you have conducted the daily health self-check and, if so, you will then be provided with the card on heavy paper stock for your use.
