Brookhaven Lab Guest Form
Please use the information below to assist you in completing some of the required fields in the Brookhaven Lab (BNL) Guest Form. If you have a current BNL Appointment and a valid BNL Guest Number or you have a pending BNL Guest Number (GR), it is not necessary to register in Guest Information System (GIS) again.

| Visit Justification: | Attend SCW 2020 Workshop – Brookhaven Lab's Media & Communication Office will be offering a Science Communication Workshop for summer students. They will come away from these sessions with tools to help them describe their research to family members, fellow students, future funders, and the media. |

Notice for Non-U.S. Citizens
Non-U.S. Citizens who have an active and up to date Brookhaven Lab (BNL) appointment, but whose Passport, Visa, or U.S. Citizenship and Immigration Services (USCIS) documentation have expired must contact the Guest, User, Visitor (GUV) Center between the hours of 7 a.m. to 5 p.m. Monday through Friday, to get their USCIS documentation updated.
