Brookhaven Lab Guest Form

Please use the information below to assist you in completing some of the required fields in the Brookhaven Lab Guest Form. If you have a current Brookhaven Lab appointment and a valid Brookhaven Lab Guest Number or you have a pending Brookhaven Lab Guest Registration (GR) Number, it is not necessary to complete Brookhaven Lab Guest Form again.

Visit Justification: BNL Main Gate Registration for the 2023 NY Wildfire Academy – The academy's goal is to provide for a safe learning environment for Wildland Firefighters and Incident Managers and to foster greater networking opportunities and partnerships between participants from federal, state, and local agencies. This workshop also supports the Laboratory's Natural Resource Management Group.

Important Notice: All participants must complete an additional Brookhaven Lab Guest Form and be approved in the BNL Guest Information System (GIS) if they would like to attend this event. Please due so by September 30, 2023. Completing the Brookhaven Lab Guest Form does not automatically register you for this event.

Notice for Non-U.S. Citizens

Attendees will not be allowed onsite if their Brookhaven Lab appointment is expired!

Non-U.S. citizens who have a valid and up to date Brookhaven Lab appointment, but whose Passport, Visa, or U.S. Citizenship and Immigration Services (USCIS) documentation have expired must bring their updated paperwork with them and check-in at the Guest, User, Visitor (GUV) Center between the hours of 7 a.m. to 5 p.m. Monday through Friday to update their USCIS documentation.

Upon arrival at BNL, please go to the GUV Center located in the Research Support Building (Bldg. 400) to check-in. Please make sure to bring proper identification.
