Please use the information below to assist you in completing some of the required fields in the Brookhaven Lab Guest Form. If you have a current Brookhaven Lab appointment and a valid Brookhaven Lab Guest Number or you have a pending Brookhaven Lab Guest Registration (GR) Number, it is not necessary to complete Brookhaven Lab Guest Form again.

Important Notice: All participants must complete the additional Brookhaven Lab Guest Form and be approved in the BNL Guest Information System (GIS) if they would like to attend this event. Please due so by September 20, 2022. Completing the Brookhaven Lab Guest Form does not automatically register you for this event.
Notice for Non-U.S. citizens
Non-U.S. citizens who have an active and up to date Brookhaven Lab appointment, but whose Passport, Visa, or U.S. Citizenship and Immigration Services (USCIS) documentation have expired must contact the Guest, User, Visitor (GUV) Center between the hours of 7 a.m. to 5 p.m. Monday through Friday, to get their USCIS documentation updated.
