Brookhaven Lab Guest Form

Please use the information below to assist you in completing some of the required fields in the Brookhaven National Laboratory (BNL) Guest Form. If you have a current BNL Appointment and a valid BNL Guest Number or you have a pending BNL Guest Registration (GR) Number, it is not necessary to register in Guest Information System (GIS) again.

Important Notice: All participants must complete the Brookhaven Lab Guest Form by January 25, 2021 and be approved in the BNL Guest Information System (GIS) in order to attend this workshop.

Notice for Non-U.S. Citizens

Non-U.S. citizens who have an active and up to date Brookhaven Lab appointment, but whose Passport, Visa, or U.S. Citizenship and Immigration Services (USCIS) documentation have expired must contact the Guest, User, Visitor (GUV) Center between the hours of 7 a.m. to 5 p.m. Monday through Friday, to get their USCIS documentation updated.
