Brookhaven Lab Guest Form
Important Notice: All attendees must complete the BNL Guest Registration form by October 31, 2016 and be approved in the BNL Guest Information System (GIS) if they would like their travel and transportation expenses reimbursed. In addition, all attendees cannot be reimbursed if their BNL Employee/Guest status is inactive (appointment has expired).
Please use the information below to assist you in completing some of the required fields in the Brookhaven Lab (BNL) Guest Form. If you have a current BNL Appointment and a valid BNL Guest Number or you have a pending BNL Guest (GR) Number, it is not necessary to register in GIS again.

Notice for Non-U.S. Citizens
Non-U.S. Citizens who have an active (up to date) Brookhaven Lab (BNL) appointment, but whose Passport, Visa, or USCIS documentation have expired must contact the Guest, User, Visitor (GUV) Center between the hours of 7 a.m. to 5 p.m. Monday through Friday, to get their USCIS documentation updated.
