Brookhaven Lab Guest Form

Important Notice: All guests must complete an additional Brookhaven Lab Guest Information System (GIS) form by January 5, 2017 and be approved before they can use BNL's onsite accommodations. Registrations received after the deadline may not receive approval in time to stay onsite.

Registering in GIS does not automatically register you for this event.

Please use the information below to assist you in completing some of the required fields in the Brookhaven Lab (BNL) Guest Form. If you have a current BNL Appointment and a valid BNL Guest Number or you have a pending BNL Guest (GR) Number, it is not necessary to register in GIS again.

Visit Justification: Attend the RBRC Workshop: QCD in Finite Temperature and heavy-Ion Collisions - The purpose of this workshop is to gather experts to identify open theoretical issues/opportunities and share ideas on future directions of jet quenching & energy loss of high energy probes; heavy quark transports & heavy quarkonia; electromagnetic probes of photons & di-leptons; thermalization & turbulence, and the QCD critical point.

Notice for Non-U.S. Citizens

Attendees will not be allowed on-site if their Brookhaven Lab (BNL) appointment is expired!

Non-U.S. Citizens who have an active (up to date) BNL appointment, but whose Passport, Visa, or USCIS documentation have expired must bring their updated paperwork with them and check-in at the Guest, User, Visitor (GUV) Center between the hours of 7 a.m. to 5 p.m. Monday through Friday to update their USCIS documentation.

Upon arrival at BNL, please go to the GUV Center (www.bnl.gov/guv/) located in the Research Support Building (Bldg. 400) to check-in. Please make sure to bring proper identification (www.bnl.gov/guv/id.php).
